The John Palermo D-Line Skills Camp – General Release Agreement
IMPORTANT NOTICE:
By completing registration for The John Palermo D-Line Skills Camp, the participant (or someone acting on their behalf) acknowledges that a parent or legal guardian has reviewed, understood, and accepted the terms of this agreement. Even if the parent or guardian does not physically sign this form, submission of registration for a minor serves as confirmation that they have granted consent and that no party associated with the camp can be held liable in the absence of a physical signature.
THE PARENT OR LEGAL GUARDIAN OF THE REGISTERED ATHLETE MUST READ AND ACCEPT THE FOLLOWING TERMS IN ORDER FOR THE ATHLETE TO PARTICIPATE IN THE CAMP.
As a participant (or parent/legal guardian of a participant) in The John Palermo D-Line Skills Camp, held on June 28, 2025, at the Pro Football Hall of Fame Fields in Canton, Ohio, I acknowledge and understand that participation in this athletic camp carries certain inherent risks of injury, illness, or other harm.
Liability Waiver
In consideration for being permitted to attend and participate in this Camp, I, on behalf of myself, my child, and our respective heirs, executors, and assigns, hereby release and discharge The John Palermo D-Line Skills Camp, John Palermo, camp staff, volunteers, and any associated organizations or sponsors (collectively, “Releasees”) from any and all liability for injury, illness, death, property damage, or other loss suffered by the participant during or as a result of participation in the Camp, whether caused by negligence or otherwise.
Medical Consent
I authorize the Camp staff to act in their best judgment in the event of a medical emergency. I consent to any necessary medical treatment for the participant, including evaluation, treatment, or transport to a medical facility. I agree to bear any costs associated with such treatment.
Media Release
For good and valuable consideration, I grant permission to The John Palermo D-Line Skills Camp to use the name, likeness, image, voice, and/or appearance of the participant in any photographs, video recordings, digital images, or other media (collectively, “Materials”) taken in connection with the Camp for promotional, advertising, and educational purposes, in any and all media now or later developed, worldwide and in perpetuity, without further compensation or notification.
I waive any right to inspect or approve any finished product that includes the participant’s likeness or any written or spoken copy that may be used in connection with the Materials.
Covenant Not to Sue
I covenant and agree not to initiate or pursue any legal action or claim against the Releasees for any matter released and discharged in this agreement.
Acknowledgment and Agreement
I have read and fully understand the terms of this General Release Agreement. I acknowledge that I have the legal authority to sign this release on behalf of the participant and that I am doing so voluntarily and knowingly, with full understanding of the risks involved.
By checking the box labeled “I AGREE” during registration, I confirm my acceptance of these terms and conditions.
Comments

Media

Who we share your data with

How long we retain your data
